Replacement of Passport (Mutilated/Damaged)

* Application for Passport for an adult [FORM PP1]
* Application for Passport for a Child Under 16yrs [FORM PP2] |
- Cash (South African Rands ONLY)
- Bank Guaranteed Cheques (to the Kenya High Commission) OR
- Postal Order (to the Kenya High Commission). Postal orders can be obtained from any Post Office

- Application letter comprehensively explaining when, where and the circumstances under which the passport got mutilated.
- Two copies of Sworn Affidavit done by Commissioner for Oath declaring the passport is mutitaled.
- Two copies of fully and correctly completed prescribed application form (PP1 for adults over 16 or PP2 for Minors below 16.)
- Four passport- sized photographs (size 2"X 1.5") with one photo signed at the back by your recommender ( Refer to note nos. 2& 3 in the application form)
- Two copies of your Immigration Status in South Africa (i.e. Permanent Residence, Work Permit, Student Authorization etc).
- Additional documentary proof of Kenya citizenship may be required if necessary
